Dental Implant Aftercare — Day-by-Day Recovery Guide

Detailed aftercare protocol for dental implant patients. Day-by-day instructions for the first 14 days, plus long-term maintenance to maximise implant survival over decades.

The first 14 days after dental implant surgery are the most critical period for implant survival. After that, the implant continues to integrate with bone for 3 months, but the surgical site has stabilised. This guide covers exactly what to do, what to avoid, and when to contact us during the recovery period.

Day of Surgery (Day 0)

After your implant surgery, you leave the clinic with:

  • A bite-pack on the surgical site (gauze)
  • Local anaesthesia still active for 2-4 hours
  • A take-home aftercare kit with all needed supplies
  • Written instructions in your language
  • Direct WhatsApp contact for our coordinator and treating doctor
  • Immediate care (first 6 hours):

  • Bite gently on the gauze for 60 minutes, then change. Repeat 2-3 times until bleeding stops.
  • Apply ice pack to the cheek over the surgical area — 20 minutes on, 20 minutes off, for the first 6 hours. This significantly reduces swelling.
  • Keep your head elevated — sit upright or use 2-3 pillows when lying down.
  • Drink lots of cool water (room temperature, not ice-cold for the first day).
  • Take prescribed antibiotics with food. Take painkillers (paracetamol or ibuprofen) before the local anaesthesia wears off — don't wait for pain.
  • What NOT to do day 0:

  • Don't rinse, spit, or use mouthwash for 24 hours
  • Don't eat anything hot until anaesthesia wears off (you can burn your mouth without realising)
  • Don't drink through a straw (suction can dislodge the blood clot)
  • Don't smoke (this is non-negotiable — significantly impairs healing)
  • Don't drink alcohol
  • Don't exercise
  • Day 1-2: Maximum Swelling Period

    Swelling typically peaks 36-48 hours post-surgery. This is normal and expected. The cheek over the surgical area may be visibly swollen and slightly bruised — this is not a problem.

    Care:

  • Continue ice pack regimen (20 on / 20 off) for the first 36 hours
  • After 36 hours, switch to warm compress (relieves stiffness)
  • Sleep with head elevated
  • Soft food only — soup, mashed potatoes, smoothies, scrambled eggs, yogurt
  • Hydrate aggressively — at least 2 litres of water per day
  • Continue antibiotics (full course, even if you feel fine)
  • Pain managed with painkillers as needed
  • Mouth care:

  • Day 1: still no rinsing
  • Day 2: gentle salt-water rinse (1/2 teaspoon salt in 1 cup warm water) — swish gently, don't spit forcefully, just let the water drain out
  • Day 3-7: Active Healing

    Swelling reduces noticeably from day 3 onwards. By day 7 most external swelling is gone.

    Daily routine:

  • Salt-water rinse 3-4 times per day after eating
  • Gentle brushing of unaffected teeth (avoid the surgical site directly)
  • Soft toothbrush only (provided in your aftercare kit)
  • Soft to medium-soft food — pasta, fish, well-cooked vegetables, soft bread
  • Continue antibiotics through full course (typically 5-7 days)
  • Take painkillers only as needed (most patients need them only occasionally by day 4)
  • Activities allowed by day 5:

  • Light walking
  • Gentle sightseeing (we recommend Kaleiçi old town, beach walks, Antalya Museum)
  • Hotel pool (no swimming — just sitting in shallow water OK)
  • Activities still to avoid:

  • Smoking (continue avoiding through full healing — minimum 14 days post-op)
  • Alcohol (continue avoiding for first week)
  • Strenuous exercise (minimum 7 days)
  • Swimming (lakes, ocean, hotel pool — minimum 7-14 days)
  • Hot tubs, saunas (minimum 7 days)
  • Air travel (minimum 24 hours, ideally 48+)
  • Day 7-14: Suture Removal Window

    If your surgery used non-resorbable sutures, they need to come out at day 7-10. Most WeCare cases use resorbable sutures that dissolve naturally — confirm with your surgeon at discharge.

    By day 14:

  • Surgical site looks essentially normal externally
  • Internal healing continues (osseointegration phase 1)
  • Diet returns to normal except for hard/crunchy items
  • Smoking still avoided (key for implant success)
  • Week 3-12: Osseointegration

    After the first 14 days, the implant is stable but actively integrating with bone. This takes 3 months.

    Daily:

  • Normal oral hygiene with soft brush + Waterpik
  • Salt-water rinse no longer needed
  • Diet fully returned to normal
  • Avoid biting hard objects directly on the implant area (no ice cubes, popcorn kernels, hard candy)
  • Avoid:

  • Smoking (ideally entire 3 months)
  • Heavy bruxism (use night guard if you grind — provided in your aftercare kit)
  • Direct trauma to the implant area
  • Long-Term Maintenance (After Final Crown)

    Once your final crown is placed (3 months after surgery), you have a fully functional implant. Long-term care:

    Daily:

  • Brush 2× per day with soft brush
  • Floss daily — special floss threaders if implant supports a bridge
  • Daily Waterpik / oral irrigator (especially around implant collar)
  • Antibacterial mouthwash if recommended (chlorhexidine for first 3 months, switch to alcohol-free maintenance after)
  • Every 6 months:

  • Professional cleaning at WeCare or your home dentist
  • Periodontal screening around the implant
  • Photographic check of gum line
  • Every year:

  • Panoramic X-ray (or focused implant X-ray) to verify bone level
  • Full oral examination
  • Update your night guard if it shows wear
  • Every 5 years:

  • Comprehensive examination including 3D CBCT if any concern
  • Soft tissue health review around all implants
  • Crown integrity check

  • Implant area feels different (sensitivity, mobility, soreness)

  • Any change in bite or chewing comfort
  • Crown feels different or sounds different when you tap
  • Any pain, even mild, that persists more than 24 hours

    Smoking — Honest Conversation

    If you smoke, your implant has measurably higher risk of failure. WeCare's protocol for smokers includes:

  • Mandatory 2-week pre-surgery cessation
  • Mandatory 14-day post-surgery non-smoking
  • Strongly recommended 3-month complete cessation through osseointegration
  • Long-term: peri-implantitis screening every 4-6 months instead of every 12
  • If you cannot quit, vape with nicotine patches is significantly less harmful than cigarettes for implant healing. Discuss this honestly during your treatment plan — we will not judge, but we want you to know the trade-offs.

    Travel Day

    If you are flying home day 6-7 post-implant surgery:

  • Take painkiller before flight (cabin pressure can cause minor discomfort)
  • Bring emergency aftercare kit in carry-on
  • Inform cabin crew if you have any concern
  • Keep hydrated during flight

    When can I fly home after implant surgery?

    Most patients fly home 2-5 days post-surgery (day 6-7 of the trip). 24 hours minimum after surgery, ideally 48-72 hours for swelling to peak and decrease. Flying with active swelling is uncomfortable but not dangerous. Cabin pressure does not affect implant healing.

    How much pain should I expect?

    Mild to moderate for the first 2-3 days, manageable with paracetamol or ibuprofen. By day 4 most patients need painkillers only occasionally. Severe pain that does not respond to medication is unusual — contact us immediately.

    When can I exercise after implant surgery?

    Light walking from day 5. Moderate exercise (jogging, light gym) from day 14. Heavy exercise (heavy weights, contact sports) from day 21. Avoid anything that raises blood pressure significantly during the first week to prevent bleeding.

    Can I smoke at all?

    No, ideally not for 3 months. If you absolutely cannot quit, minimum 14 days post-surgery is required for successful integration. Smokers have approximately 2× higher implant failure rate. We will treat smokers but with realistic expectations.

    When can I drink alcohol?

    Avoid completely for the first 7 days. From day 7-14, light to moderate alcohol is acceptable. Avoid heavy drinking through full integration (3 months) — alcohol impairs bone healing.

    What if my implant fails after I return home?
