Many seniors assume they are "too old" for dental implants. They are not. Age alone is not a contraindication for implant surgery. The Real Question — Health, Not Age

The implant literature is unambiguous: chronological age does not predict implant outcome. What predicts outcome is:

  1. Cardiovascular health — Patients with controlled blood pressure and stable heart conditions do well.
  2. Bone density — Osteoporosis affects healing more than age. Bisphosphonate medication needs special evaluation.
  3. Medication list — Anticoagulants, immunosuppressants, and steroids require protocol modifications.
  4. Diabetes status — Same rules apply for any age. HbA1c below 7.0% is the target.
  5. Mobility and recovery support — Travel logistics matter more for seniors. We adjust accordingly.

We have placed implants in patients up to 87 years old with excellent outcomes. The oldest All-on-4 patient at WeCare was 83 at the time of surgery and is now in year 4 of a permanent bridge.

Pre-treatment Screening

For any senior patient, we request a slightly more detailed pre-screening:

  • Recent panoramic X-ray (OPG) — standard
  • 3D CBCT scan — done in clinic on day 1
  • Full medications list — including all OTC supplements
  • Recent blood pressure readings
  • Most recent blood test results (within 6 months)
  • Cardiologist letter if you have a heart condition
  • Endocrinologist letter if you have diabetes
  • Bone density (DEXA) result if you take osteoporosis medication

Within 24 hours of receiving this, we send a written treatment plan with any modifications for senior protocol.

Bisphosphonates and Implants

Bisphosphonates (Fosamax, Boniva, Zometa, Aclasta, etc.) are commonly prescribed for osteoporosis. Long-term oral use raises a small risk of medication-related osteonecrosis of the jaw (MRONJ) after dental surgery. The risk depends on duration of use, route of administration, and underlying bone health.

Oral bisphosphonates under 4 years — Implants generally safe with standard protocol. We may recommend a "drug holiday" of 3 months pre- and post-surgery if your prescribing doctor agrees.

Oral bisphosphonates over 4 years — Risk increases. We evaluate case-by-case. May recommend bone marker blood test before proceeding.

IV bisphosphonates (Zometa, Aclasta) — Significantly higher risk. We typically advise against elective implant surgery, or recommend specialised oral surgery centres.

We will not place implants in higher-risk bisphosphonate cases without full disclosure and documented consent. Patient safety comes first.

Anticoagulant Medications

Many seniors are on blood thinners. Most can safely undergo implant surgery without stopping the medication:

  • Aspirin (low dose) — Continue without interruption
  • Warfarin (Coumadin) — Continue if INR is below 3.0; we will check on the morning of surgery
  • Direct oral anticoagulants (Eliquis, Xarelto, Pradaxa) — Continue or skip morning dose only on surgery day, in consultation with your prescribing physician
  • Clopidogrel (Plavix) — Continue without interruption

We never advise stopping anticoagulants without input from your prescribing physician. We coordinate with them when needed.

Modified Senior Protocol

Surgical

  • Local anaesthesia preferred (not IV sedation)
  • Procedure broken into shorter sessions if multiple implants
  • Gentle handling of soft tissues to reduce post-op swelling
  • Sutures removed at 10-14 days (vs 7 days for younger patients)

Recovery

  • Extended hotel stay if needed (we book whatever number of nights makes sense)
  • Lower-mobility room (ground floor or close elevator) on request
  • Daily WhatsApp check-in by our coordinator
  • Soft-food diet plan with senior-appropriate nutrition

Healing

  • Standard 3 months for most cases
  • Extended to 4 months if osteoporosis or systemic conditions

Aftercare

  • Annual remote photo screening for 5 years included
  • Free guidance for your home dentist on cleaning protocols

Should I stop my blood thinner before surgery?

Can my partner/companion stay with me at the hotel?

Will the surgery be painful at my age?

No. Implant placement is performed under local anaesthesia (the same numbing used for fillings). Most senior patients report no pain during the procedure and only mild swelling for 3-5 days afterward, manageable with paracetamol or ibuprofen as your physician approves.

How long is the hotel stay for senior patients?
