When dental implants require bone reconstruction (which is common after long-standing tooth loss), the choice of grafting material matters as much as the implant brand itself. Bio-Oss® from Geistlich Pharma (Wolhusen, Switzerland) is the most-researched and most-used bone graft material in modern implantology, with over 30 million successful procedures performed worldwide and 1500+ peer-reviewed clinical publications. What Is Bio-Oss®?

Bio-Oss® is a deproteinised bovine bone matrix — natural bone particles harvested from cattle, processed to remove all organic and protein content while preserving the calcium phosphate mineral framework. The result is a sterile, biocompatible scaffold that:

  • Mimics natural human bone microstructure exactly
  • Provides a porous lattice for your own bone cells to grow into
  • Resorbs slowly over years (unlike synthetic substitutes that resorb in months), preserving graft volume long-term
  • Has zero risk of disease transmission (extensively processed and sterilised)
  • Is fully biocompatible with no documented allergic reactions
  • When Bio-Oss® Is Used

    We use Bio-Oss® for several specific clinical situations:

    1. Socket preservation — At the moment of tooth extraction, Bio-Oss® particles are placed in the socket to preserve bone volume for future implant placement. Without this, the socket can shrink up to 50% in the first year.

    2. Ridge augmentation — When the alveolar ridge has shrunk after tooth loss, Bio-Oss® rebuilds the ridge in width or height. Often combined with a Geistlich Bio-Gide® collagen membrane to guide bone regeneration.

    3. Sinus lift — For upper-jaw molar implants where the sinus has expanded into available bone, Bio-Oss® fills the lifted sinus floor space. Both crestal (closed) and lateral (open) sinus lift techniques use Bio-Oss®.

    4. Peri-implant defect repair — When existing implants have lost surrounding bone, Bio-Oss® reconstructs the defect.

    Bio-Oss® vs Synthetic Alternatives

    Synthetic bone graft materials (β-TCP, hydroxyapatite, bioactive glass) are commercially available and cheaper. They have a place in clinical practice but with measurable trade-offs:

  • Resorption — Synthetic graft typically resorbs in 4-12 months. Bio-Oss® resorbs slowly over years, preserving graft volume.
  • Bone integration — Synthetic graft usually integrates well, but the histological similarity to natural bone is lower than Bio-Oss®.
  • Long-term volume stability — Bio-Oss® has documented decade-plus stability in published literature.
  • Cost — Synthetic graft is more accessible. We use it sometimes when graft volume is small and the case is clinically straightforward.
  • For sinus lifts, large ridge augmentations, and any case where long-term volume stability is critical, we use Bio-Oss®.

    Bio-Oss® + Bio-Gide® — The Complete System

    Geistlich also makes Bio-Gide®, a resorbable collagen membrane that is placed over the Bio-Oss® graft to guide bone regeneration. The membrane:

  • Prevents soft-tissue from invading the bone graft area
  • Provides a barrier for new bone to form underneath
  • Resorbs over 6 months as new bone matures
  • Eliminates the need for membrane-removal surgery

    Completely safe. Bio-Oss® is processed to remove all organic and protein content, leaving only the mineral framework. The processing eliminates any disease transmission risk.     How long does Bio-Oss® take to integrate?

    4-6 months for routine cases (socket preservation, small ridge augmentation). 6 months for sinus lift. Bio-Oss® resorbs slowly — most graft material is still visible in the area for years, gradually being replaced by your own bone over time.

    Is Bio-Oss® more expensive than synthetic graft?

    Yes — Bio-Oss® is the premium-tier graft material. The clinical advantages (slow resorption, long-term volume stability, extensive research base) justify the difference for major cases. For small grafts in straightforward cases, synthetic substitutes may be acceptable. We discuss the right choice for your case during planning.

    What if I have a religious objection to bovine-derived material?

    We respect that and offer alternatives. Synthetic graft materials (β-TCP, hydroxyapatite) are available as alternatives, as is allograft (human donor bone). Tell us during planning and we adjust the treatment protocol.

    Will my implant be placed at the same time as the graft?

    Sometimes. For small grafts (socket preservation, minor ridge augmentation), the implant can be placed simultaneously. For large grafts (lateral sinus lift, severe ridge augmentation), we typically stage: graft first, healing 4-6 months, then implant. CBCT analysis tells us which approach for your case.

    Will I need a second surgery to remove the membrane?

    No. Bio-Gide® is a resorbable collagen membrane — it dissolves naturally over 6 months. No removal surgery required. Older membranes (PTFE) needed removal but we use only resorbable ones.
