Bisphosphonates — drugs like Fosamax, Boniva, Aclasta, Zometa — are widely prescribed for osteoporosis and bone-related cancer. Long-term use raises a small but real risk of medication-related osteonecrosis of the jaw (MRONJ) after dental surgery, including implants. The risk depends on how long you have taken the drug, the route (oral vs IV), and the specific drug. WeCare's screening protocol determines if implants are safe for you, and adjusts the protocol when needed.

The Risk Categories

Oral bisphosphonates under 4 years use: Risk of MRONJ is low (under 0.1%). Implants generally safe with standard protocol. We may recommend a 3-month drug holiday around surgery if your prescribing physician approves.

Oral bisphosphonates 4+ years use: Risk increases. We perform bone marker blood test (CTX serum) to assess current bone turnover state. CTX above 150 pg/mL → proceed cautiously. Below 150 pg/mL → defer surgery, request 3-6 month drug holiday with prescribing physician.

IV bisphosphonates (Zometa, Aclasta) for osteoporosis: Risk is higher (around 1-2%). Case-by-case evaluation only after thorough discussion of risks and prescribing physician's input.

IV bisphosphonates for cancer (chemo regimens): Risk is significantly higher (5-10%). We typically advise against elective dental implant surgery in active cancer treatment. Conservative dental work (cleanings, fillings) only.

IV bisphosphonates (Zometa, Aclasta) are 100-1000Ă— more potent than oral forms. They accumulate in bone tissue and significantly suppress bone turnover. After dental surgery, the affected bone area can fail to heal properly, leading to medication-related osteonecrosis.

What is a CTX bone marker test?↓

C-terminal telopeptide of type I collagen — a blood test that measures bone turnover. Low CTX (under 100-150 pg/mL) indicates suppressed bone turnover from bisphosphonate use, raising MRONJ risk. Higher CTX suggests bone is actively remodelling and surgery is safer.

Can I do a drug holiday before surgery?↓

Sometimes. Drug holiday (stopping bisphosphonate temporarily) is decided in coordination with your prescribing physician — never independently. Typical protocol is 3 months pre-surgery and 3 months post-surgery, but specifics depend on your osteoporosis severity and other risk factors.

What if I have already had bisphosphonates and need an implant?↓

We assess your specific situation. Many patients on long-term oral bisphosphonates have successful implant outcomes when proper screening and modified protocol is used. Honest evaluation comes first; if the risk is too high, we recommend alternatives.

Is there an alternative to implants for me?↓

Yes. Implant-retained dentures (overdentures) sometimes use 2 implants instead of 4-6, reducing surgical exposure. Traditional dentures involve no surgery at all. Bridges using existing healthy teeth as support require no implants. We discuss all options.
